How to Renew a DMV Traffic Violator School (TVS) Owner License

The TVS owner must be licensed by DMV. In order for the TVS owner license to be issued, you must complete the traffic violator instructor application in addition to the traffic operator application.

1. Preparation

Ensure you’re familiar with the renewal process and requirements.

Have your login details for the DMV website ready.

2. Gather Necessary Documents & Forms

Required Documents:

a. Application Forms:

  • Driving and Traffic Violator School Owner Renewal Application (OL 45A)
  • Classroom/Branch Renewal Application (OL 737)

b. Lesson Plan Submission:

  • If creating an original lesson plan: Submit a Request for Approval of TVS Educational Program (OL 764).
  • If using a previously approved lesson plan: Submit a Request for Use of an Approved TVS Program (OL 766).

3. Submit Application and Documents Online

  • Log in to the DMV website.
  • Navigate to the TVS Owner License renewal section.
  • Upload the completed forms: OL 45A, OL 737, and either OL 764 or OL 766 as applicable.
  • Pay the associated fees online.

4. Fees

  • $100 non-refundable renewal application fee.
  • $1 family support program fee.
  • $15 duplicate license fee (if applicable).

Note: When applying for any combination of owner/operator/instructor license, only the owner fee will be due.
