How to Apply for a DMV Traffic Violator School (TVS) Operator License

Either the owner or an employee who is designated by the owner to direct and manage the school. A TVS operator may perform operator services for more than one school under the following conditions:

  • The licensed TVSs share a common owner.
  • The licensed TVSs share an established business address. Show Citation 11
  • The operator possesses a valid license for each school.

1. Preparation

Familiarize yourself with the requirements and processes.

2. Gather Necessary Documents & Forms

Required Documents:

a. Application Forms:

  • TVS Operator Application Checklist (OL 750)
  • Application for Traffic Violator School Operator and Instructor License (OL 710)
  • Traffic Violator School Operator Assurance of Compliance with the Americans with Disabilities Act of 1990 (OL 141)

b. If NOT submitting a TVS Owner Application:

  • Application for Occupational License Personal History Questionnaire, Part B (OL 29B)
  • Request for Live Scan Service [yellow copy] (DMV 8016)

c. Educational Training:

  • Evidence of completion of an approved 8-hour operator educational program, stamped with proof of passing examination.
  • Note: The 8-hour course is a good preparation for the TVS Operator Examination, so completing this first is recommended.

d. TVS Examination:

Evidence of successful completion of the TVS operator written examination.

e. Age Verification:

Proof of age showing you are at least 21 years old.

3. DMV Investigation

Upon receipt of an application for a license accompanied by the appropriate fee, the DMV will conduct a thorough investigation of the information provided. This process is expected to take up to 120 days.

4. Submit the Application and Take the Examination

  • Once you have completed the forms and collected all required documents, visit a DMV inspector’s office.
  • Submit your paperwork and associated fees.
  • Take the TVS examination at the DMV inspector’s office. Note: You have three attempts to pass the written examination.
  • Recommendation: Keep a copy of all documents for your records.

5. Application Timeline

Ensure you complete your application within one year. If not, a new application with all examinations and fees will be required.

6. Fees

  • $100 non-refundable application fee.
  • $1 family support program fee.
  • $16 test fee.
  • $15 duplicate or replacement license fee (if applicable).
  • $50 license transfer fee (if applicable).
